Terms & Conditions

1. Introduction
These Terms and Conditions apply to all appointments, services, and interactions provided by ADAPT. PERFORM. Physiotherapy & Performance Clinic, operated by Jonathan Scott (trading as ADAPT. PERFORM.) located at 19 Backfields Lane, St Pauls, Bristol, BS2 8QW.
By booking or attending an appointment, you agree to these terms.
2. Appointments & Payments
· Appointments can be booked online, by phone, or in person.
· Payment is required at the time of booking or on the day of your appointment.
· Accepted payment methods include debit/credit card or secure online payment.
3. Cancellations & Missed Appointments
· A minimum of 24 hours’ notice is required to reschedule or cancel an appointment.
· Cancellations made with less than 24 hours’ notice or missed appointments will incur the full session fee.
· We understand emergencies happen — please contact us as soon as possible if unavoidable circumstances arise.
4. Insurance Claims
· If you have private medical insurance, please contact your provider before booking to confirm your eligibility for either reimbursement or the following information:
Name of the insurance company
Membership Number and Authorisation Number
Number of treatments authorised by the insurance company
Excess on your insurance policy
Renewal date of your insurance policy
· ADAPT. PERFORM. does not directly invoice insurers unless recognised with them. The following are currently registered and recognised by us: Aviva, AXA, BUPA, Cigna, Healix, WPA and Vitality.
5. Sharing of Information & Confidentiality
· All client information is confidential and handled in accordance with GDPR and HCPC standards.
· We may share relevant information with other healthcare professionals involved in your care only with your consent.
· Client details are securely stored through Cliniko, our GDPR-compliant practice management system.
6. Consent to Treatment & Media Use
· By attending an appointment, you consent to assessment, treatment, and rehabilitation as discussed with your clinician.
· Occasionally, we may request consent to record parts of a session (video or photo) for educational, social media, or website purposes.
· Such recordings will only be made and shared with your explicit written consent and can be withdrawn at any time.
7. Health & Safety
· Clients are expected to disclose all relevant medical information before and during treatment.
· ADAPT. PERFORM. accepts no responsibility for injury or aggravation resulting from undisclosed conditions or non-adherence to guidance.
8. Limitation of Liability
· While every effort is made to provide safe and effective treatment, ADAPT. PERFORM. cannot guarantee specific outcomes.
· Liability for any damages shall not exceed the cost of the individual session.
9. Business Continuity
In the event of unexpected clinic closure, illness, or practitioner unavailability, patients will be contacted as soon as reasonably possible to rearrange appointments or discuss alternative arrangements.
10. Refunds
· Refunds are not available for completed sessions.
· Packages or prepaid services may be refunded only in exceptional circumstances at the discretion of the clinic.
11. Complaints Procedure
We aim to provide the highest standard of care at all times.
If you are unhappy with any aspect of your treatment or service, please raise your concern directly so it can be addressed promptly and fairly. Complaints can be made verbally during your appointment or in writing by emailing info@adaptperform.co.uk.
If a complaint cannot be resolved informally, you have the right to escalate the matter to the Health and Care Professions Council (HCPC), the statutory regulator for physiotherapists in the UK.
